Membership Application
(I) Ordinary Members
All full-time, regular service officers and civilian personnel serving the seven Government Disciplined Services Departments (including Correctional Services Department, Customs & Excise Department, Fire Services Department, Government Flying Service, Hong Kong Police Force, Immigration Department, Independent Commission Against Corruption), shall be eligible to be Ordinary member. Auxiliary officers or part-time civilian staffs, irrespective of ranks, are not eligible to be ordinary members.
(II) Family Members
Spouses and children of married Ordinary Members are eligible to be Family Members. Ordinary Members' children who are married or have reached the age of 21 shall cease to be members unless they have acquired membership in their own right. All Children Family members are required to renew their membership every five years.
(III) Retired Members
Regular service members of the disciplined services who have retired at the normal age of retirement on completion of their normal service under the Old Pension Scheme, the New Pension Scheme or the Civil Service Provident Fund (CSPF) Scheme and the spouse of married retired members are eligible to be Retired Members. Civilian personnel retired from the disciplined services are not eligible to be retired members.

Those regular service members of the disciplined services who have completed 18 years of service and have served on agreement terms and left the discipline services on satisfactory completion of agreement, and the spouse of married retired members, are eligible to be retired members.

Those regular service members of the disciplined services who have completed 18 years of service and have retired pre-maturely on the Old Pension Scheme, the New Pension Scheme or the CSPF Scheme, or been invalidated out of the service are also eligible to be retired members, provided that their applications are supported by the respective Disciplined Services Departments concerned and endorsed by the Executive Committee.
(IV) Associate Members
Individuals who have contributions to the DSSRC may be elected Associate Members by the Executive Committee depending on merits of individual cases. Such membership, although carrying entitlement to full usage of the Club's facilities, is restricted to individuals, and may not extend to their family members. Associate Members are required to renew their membership annually.
(V) Temporary Members
All full-time non-civil service contract staff, serving within the government disciplined services departments, shall be eligible to apply for "Temporary" membership, subject to the term of his / her contract with the government is not less than twelve calendar months from the date of his / her application for membership and endorsed by the head of his / her department. Such membership, although carrying entitlement to full usage of the Club's facilities, is restricted to individuals, and may not extend to their family members.
Collection of Personal Data
1. Purpose of Collection
The information provided in the Membership Application Form will be used by the DSSRC to facilitate the processing of membership cards and communication with members. Provision of personal data is voluntary.
2. Classes of Transferees
The personal data you provided may be disclosed to relevant departments of disciplined services for the purpose mentioned in paragraph (1) above.

2. Types of Membership
1. The Club shall consist of the following types of members
a.Ordinary Members
All full-time, regular service officers and civilian personnel serving within the seven Government
Disciplined Services Departments listed below shall be eligible to be Ordinary member, namely (in
alphabetical order):
• Correctional Services Department
• Fire Services Department
• Hong Kong Police Force
• Independent Commission Against Corruption
• Customs & Excise Department
• Government Flying Service
• Immigration Department
※ Auxiliary officers or part-time civilian staff, irrespective of ranks, are not eligible to be ordinary
members.
b.Family Members
Spouse and children of married serving full-time, regular service officers and civilian personnel of
Ordinary Members specified at Rule (2.1.a) above are eligible to be Family Members. Ordinary
Members’ children who are married or have reached the age of 21 shall cease to be members unless
they have acquired membership in their own right. All Children Family members are required to renew
their membership every five years.
c.Retired Members
i.Regular service members of the disciplined services who have retired at the normal age of
retirement on completion of their normal service under the Old Pension Scheme, the New Pension
Scheme or the CSPF Scheme and the spouse of married retired members are eligible to be
Retired Members. Civilian personnel retired from the disciplined services are not eligible to be
retired members.
ii.Those regular service members of the disciplined services who have completed 18 years of service
and have served on agreement terms and left the discipline services on satisfactory completion of
agreement, and the spouse of married retired members, are eligible to be retired members
iii.Those rdeegular service members of the disciplined services who have completed 18 years of
iii.service and have retired pre-maturely on the Old Pension Scheme, the New Pension Scheme or the

iii.provided that their applications are supported by the respective Disciplined Services Departments

iv.Regular service members of the disciplined services who have retired from Hong Kong Police Force
iv.or Fire Services Department, are eligible to be Retired Members. The Applicant can present the
iv.Application Form, 2 photos, Certificate of service copy and the identification copy to each Welfare
iv.Department or Hong Kong Police Recreation Club to make the endorsement. Regular service
iv.members of the disciplined services who have retired from other disciplined services, are eligible to
iv.be Retired Members. Applicant can present the Application Form, 2 photos, Certificate of service
iv.and the identification copy to its department Headquarter to make the endorsement.
v.The spouse of married retired members, are eligible to be retired members. Regular service retired
v.member who have retired from Hong Kong Police Force or Fire Services Department, are willing to
v.apply the membership for his spouse, can present the Application Form, Marriage Certificate copy,
v.the spouse's identification copy and 2 photos to each Welfare Department or Hong Kong Police
v.Recreation Club to make the endorsement. Regular service retired members who have retired from
v.other disciplined services, are willing to apply the membership for his spouse, can present the
v.Application Form,Marriage Certificate copy, the spouse's identification copy and 2 photos to its
v.department Headquarter to make the endorsement.
d.Temporary Members

3.Application Fees
1.New application and Renewal: HK$12 per card; Loss, Name Change or Replacement: HK$30 per card for the
first time, thereafter HK$50 per card.
2.The application fees must be submitted together with the application forms.
4.Payment Methods
1.If the endorsed application form is sent to the Club via disciplined service departments
a.Payment by cheque should be made payable to the "Spotlight Enterprises Limited". Please mark the
name of applicant, the department of disciplined service and the contact telephone number
clearly on the back of the cheque and attach the cheque on the left hand corner on the cover page of the
application form.
b.The applicant can deposit the amount into the "Hong Kong and Shanghai Banking Corporation Limited"
current account (002-598381-002), and attach the original copy of the bank receipt on the cover page of
the application form.
2.If the endorsed application form is submitted directly to the Club
a.The applicant can either pay by cash or by cheque (Payable to the "Spotlight Enterprises Limited" ) at the
Club Office.
5.Application Notes
1.Please attach 2 photos and the application fees when submitting the application form to the Club.
2.The applicant must fill in all the information on Section A&B and sign the signature on Section C before
submitting the application form.
3.To protect the privacy of members, if applicant do not collect his/her membership card within one year after
the submission of application, the Club Management reserves the right to destroy all relevant membership
card(s) and document(s) without prior notice. The fee paid will not be refunded.
4.The applicant must fill in clear personal data on the application form. The club will not shoulder any
responsibility if present the mistake on causing its membership card because of the applicant fails to offer the
clear personal data.
5.The applicant must pay attention for Column D. The section must be chopped by relevant department and the
applicant must submit the application form to relevant department for endorsement in person before
submitting it to the Club. This column must be filled in by The Head of Department or an officer appointed by
him.
